All services are operated by Trin’Naz Helping Hands, a nonprofit community-based organization. Program availability, duration, and scope may vary based on funding, staffing capacity, eligibility requirements, and community need.
Services may be delivered in person, virtually, or within community settings, including schools, shelters, partner locations, or client homes, depending on program design and participant needs.
Enhanced Care Management is a statewide Medi-Cal benefit designed to support individuals with complex medical, behavioral health, and social needs. ECM services are provided in alignment with Medi-Cal guidelines and emphasize whole-person, coordinated care.
Participation in ECM is voluntary and subject to eligibility determination. ECM services focus on care coordination, advocacy, and connection to services and do not replace medical, mental health, or dental providers.
Enrollment in ECM does not guarantee access to all services offered by Trin’Naz Helping Hands.
Victims of Crime Services are delivered in accordance with applicable federal and state funding requirements, including VOCA guidelines. Services are trauma-informed and may include advocacy, case management, crisis support, referrals, and short-term supportive assistance.
Emergency shelter, temporary housing, relocation assistance, and related supports are subject to availability, safety considerations, eligibility criteria, and funding capacity. Services may be time-limited and are not guaranteed.
Youth programs operate under a trauma-informed, family-engaged framework aligned with Enhanced Care Management and Children and Youth Behavioral Health Initiative (CYBHI) priorities.
Parent or legal guardian participation is required for enrollment. Participation may occur in person, by phone, or virtually to reduce access barriers.
Programs may include free meals for eligible families. Additional fee-based services may be available for youth who do not meet eligibility criteria. Program schedules, services, and activities may vary by season and funding source.
Eligibility for services is determined by program criteria, funding requirements, and individual circumstances. Participation in one program does not guarantee eligibility for additional services.
Trin’Naz Helping Hands does not discriminate based on race, color, religion, gender identity, sexual orientation, national origin, disability, or immigration status.
Trin’Naz Helping Hands partners with school districts, community-based organizations, service providers, housing partners, and public agencies to expand access to care.
Partnerships do not imply financial support, operational control, or endorsement unless defined by a formal written agreement.
